insurance, Medicare/Medicaid, and offer affordable sliding scale

Primary Care
General practice, family practice, sickness visits, injury visits, etc. Services may include annual checkups, wellness exams, chronic condition management, health screenings, immunizations, employment exams, etc. After hours or extended hours care may be available. Walk ins may be accepted.
Women’s Health
Gynecological exams and screenings. Mammography may be available. Some locations may limit care to Obstetric services.
Behavioral Health
Psychological counseling and/or Psychiatric treatment services. May offer medication management.
Family Planning
Birth control/contraceptive services.
Addiction Services
Counseling and/or treatment for substance abuse. May offer medication-assisted or inpatient treatment.
Pediatrics
Children’s healthcare. Services may include school exams or immunizations.
Dentist
Adult and Pediatric dental locations may be separate.
Pharmacy
Retail pharmacy or medication delivery services. May be patients only.
Optometrist
Eye exams. Services may include an optical shop.
Chiropractor

Foot care
HIV Care
Care for people living with HIV. May offer Ryan White Program.
PrEP
PrEP program services. PEP may be available.
Testing
Testing for HIV, STI, or HepC. Walk-in testing may be offered. Testing for other conditions may be available.
Inclusive
All CHCs are welcoming. Some may assert LGBTQ+ outreach. Gender-affirming care for Trans people may be available.
Walk-in Clinic
Walk-in sickness or injury clinic. May be an Urgent Care clinic. May offer after-hours or 24/7 care. CONTACT CHC TO CONFIRM SERVICES.
Wellness Center
Services may include exercise programs, nutrition and wellness programs, fitness center, etc. Facility may be a mixed-use community center.
Focus
CHC or a CHC location may Focus on services sensitive to and specifically to meet the needs of a group or a condition. Groups may include teenagers or young adults, Native peoples, religious groups, ethnic groups, undocumented individuals, homeless individuals, LGBTQ+, transgender individuals, farm workers, immigrants and refugees, sex workers, and others. Conditions may include HIV care; Hepatitis C; black lung; crisis, inpatient, or recovery care for opioid addiction or other substance addictions; and others.
