General practice, family practice, sickness visits, injury visits, etc. Services may include annual checkups, wellness exams, chronic condition management, health screenings, immunizations, employment exams, etc. After hours or extended hours care may be available. Walk ins may be accepted.

Gynecological exams and screenings. Mammography may be available. Some locations may limit care to Obstetric services.

Services may include exercise programs, nutrition and wellness programs, fitness center, etc. Facility may be a mixed-use community center.

CHC or a CHC location may Focus on services sensitive to and specifically to meet the needs of a group or a condition. Groups may include teenagers or young adults, Native peoples, religious groups, ethnic groups, undocumented individuals, homeless individuals, LGBTQ+, transgender individuals, farm workers, immigrants and refugees, sex workers, and others. Conditions may include HIV care; Hepatitis C; black lung; crisis, inpatient, or recovery care for opioid addiction or other substance addictions; and others.
